On or about May 23, 2019, a recordable injury occurred when an Apprentice Pipefitter Service Technician sustained avulsion injuries to the tips of the middle, ring and pinky fingers on the right hand when attempting to stop a moving fan belt on an air handler being serviced, resulting in 45 days of lost time.  The information in Column F was not recorded in the detail required, as it did not describe the object that directly injured the employee.
