29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:    At the jobsite, on or about June 2, 2019, employees were exposed to fall and struck-by hazards when an elevated work platform on the forks of a JLG G6-42A rough terrain forklift was moved horizontally while occupied and was not attached to the forks.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l)  (Construction Reference 1926.602(d)):    At the jobsite, on or about June 2, 2019, where employees were exposed to struck-by and crushed-by hazards when operating a JLG G6-42A rough terrain forklift without ensuring employees completed forklift training.      The BRW Metal Industries was previously cited for a violation of this occupational safety and health standard 1910.178(l)(1)(i) which was contained in OSHA inspection number 1215662, citation number 1, item number 3, and was affirmed as a final order on November 17, 2017 with respect to a workplace located at 7718 Lettie Street, Houston, TX 77075.
