29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  At a worksite located at 5848 Rockhill Rd, Kansas City, MO: Employees were exposed to struck-by hazards. Employees were observed using pneumatic nail guns without the use of protective eyewear.

29 CFR 1926.404(b)(1)(iii)(C): Where an assured equipment grounding conductor program was being utilized, the employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were not visually inspected fofore each day's use for external defects, such as deformed or missing pins or insulation damage, and for indicationsof possible internal damage.  At the jobsite located at 5848 Rockhill Rd, Kansas City MO:  Employees engaged in residential framing activities were exposed to an electrical shock hazard.  The employer failed to protect employees from electrical shock hazards by allowing the use of flexible cords and cables with visible damage. This was demonstrated when a RYOBI brand circular (Model # CBS135L) saw was connected to an extension cord and was found to have multiple areas of damage to the extension cord.

Three employees were engaged in roofing work and were not protected from falling by the use of guardrail systems, safety net system, or personal fall arrest system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   A worksite site located at 5848 Rockhill Rd, Kansas City, MO: Employees were exposed to a fall hazards. Employees were using Werner brand extension ladders to access the roof of the residential worksite. The extensions ladders were not extended at least 3 feet above the upper landing surface. One ladder was placed at the eve of the roof and the other was only placed one foot above the upper landing of the roof.
