29 CFR 1910.213(c)(2): Hand-fed circular ripsaw(s) were not furnished with a spreader to prevent material from squeezing the saw or being thrown back on the operator: (a) Facility, 310 Unity Street, Latrobe, PA: On or about May 30, 2019 the unlabeled 14 inch table saw was not equipped with a spreader to prevent material from squeezing the saw or being thrown back on the operator. An employee was cutting a piece of wood approximately 1 1/2 inches thick by 18 inches long x 12 inches wide when it was thrown back and struck the employee in the abdomen. The employee died the following day during surgery to repair injuries incurred from being struck.

29 CFR 1910.213(c)(3): Hand-fed ripsaw(s) did not have non kickback fingers or dogs to oppose the thrust or tendency of the saw to pick up the material or to throw it back toward the operator: (a) Facility, 310 Unity Street, Latrobe, PA: On or about May 30, 2019 there were no non-kickback fingers or dogs installed on the unlabeled 14 inch table saw. An employee was operating the saw cutting a piece of wood that measured approximately 1 1/2 inches thick by 18 inches long by 12 inches wide when it was thrown back and struck the employee in the abdomen. The employee died the following day during surgery to repair injuries sustained from being struck. (b) Facility, 310 Unity Street, Latrobe, PA: On or about June 3, 2019 there were no non-kickback fingers or dogs installed on the Delta table saw used by employees to cut wood to oppose the thrust or tendency of the saw to pick up the material to prevent it from being thrown back at the employee during operation.

(b) Facility, 310 Unity Street, Latrobe, PA: On or about June 3, 2019 there was no guard installed on the Delta 40 table saw used to cut wood for patterns. The blade was set at approximately 2 inches above the table and no guard was in place to prevent contact during operation.

29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident: (a) Facility, 310 Unity Street, Latrobe, PA: On or about June 3, 2019 the employer had not reported the death of an employee resulting from a work-related incident. On May 30, 2019 an employee sustained abdominal injuries when struck with a board. The employee died on May 31, 2019 during surgery to repair injuries sustained on May 30, 2019. The employer did not report the death to the Pittsburgh Area Office until June 3, 2019.
