29 CFR 1910.28(b)(1)(i): Each employee on a walking/working surface having an unprotected side or edge which was four feet (1.2m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.                        On 6/4/19, at the Macon facility, maintenance employees were exposed to fall hazards over 7 ft. as they worked on the top of water chillers. On 5/30/19, a maintenance employee received an electrical shock while working on the top of the chiller and fell 7 ft. 3 in. to ground levels.

29 CFR 1910.147(c)(6)(i)(C): Where lockout was used for energy control, the periodic inspection did not include a review, between the inspector and each authorized employee, of that employee's responsibilities under the energy control procedure being inspected.    On 6/4/19, at the Macon facility, the employer did not ensure the periodic inspection of the Lockout program included all authorized employees.

29 CFR 1910.333(a)(2): Where exposed live parts were not deenergized (i.e., for reasons of increased or additional hazards or infeasibility), other safety-related work practices were not used to protect employees who could be exposed to the electrical hazards involved.    On 5/30/19, at the Macon facility, maintenance employees were trouble shooting fans that were not deenergized and did not use other protective measures during that work. An employee made contact with the live equipment and received an electrical shock.
