29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a. On or about June 3, 2019, at the above addressed jobsite, an employee using a pneumatic nail gun did not wear safety glasses, thereby exposing the employee to the hazards associated with eye injuries.  b. On or about June 3, 2019, at the above addressed jobsite, an employee using a battery-operated, portable radial saw did not wear safety glasses, thereby exposing the employee to the hazards associated with eye injuries.

On or about June 3, 2019, at the above addressed jobsite, employees performing residential roofing operations, eighteen (18') feet eight (8") inches above the ground, were not protected from falls, thereby exposing employees to fall hazards.
