29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toe-boards, safety net systems, or personal fall arrest systems:    a.  On or about April 23, 2019, at the above addressed jobsite, employees conducting roofing operations, three (3) stories from the ground, were not protected from falling.

On or about April 23, 2019, at the above addressed jobsite, employees had been provided fall protection training, however, the employer failed to keep a written certification of record.
