29 CFR 1910.67(c)(2)(v): A personal fall arrest or travel restraint system that meets the requirements in subpart I of this part was not worn and attached to the boom or basket when working from an aerial lift: On or about June 1, 2019, the employer did not ensure that a personal fall arrest or travel restrain system was worn and attached to the basket when working from an aerial lift. A worker was exposed to fall hazards of up to 25 feet while conducting window cleaning activities from a Kesla aerial lift, model Scanlift SL 185, S/N 19713 without the benefit of a fall protection system. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").
