29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):  Warehouse, Swing-Arm Lamp, Doors 4/5:  On or about May 28, 2019 and at times prior thereto, the extension cord used in lieu of fixed wiring had not been visually inspected before use on any shift.  Initial inspection revealed missing ground pin.  This condition exposed employees to electrocution hazards.

29 CFR 1910.305(a)(2)(x): Flexible cords and cables were not protected from accidental damage, as might be caused, for example, by sharp corners, projections, and doorways or other pinch points:  Warehouse, Swing-Arm Lamp, Doors 2/3:  On or about May 28, 2019 and at times prior thereto, the outer jacket of the flexible cord was damaged.  This condition exposed employees to an electrical shock hazard to the 120 volt circuit from continuous wear on the swing arm of the lamp.

This condition subjected the flexible cord to damage.

29 CFR 1917.43(b)(1): Modifications (such as adding counterweights) that might affect the powered industrial trucks capacity or safety were performed without either the manufacturer's prior written approval, or the written approval of a professional engineer experienced with the equipment who had consulted with the manufacturer:  Warehouse:  On or about May 28, 2019 and at times prior thereto, the employer allowed the use of fork extensions during the operation of a Mitsubishi Model F625, serial number F178-00743 powered industrial truck (forklift) without written approval.  This condition modified the capacity of the forklift.
