29 CFR 1917.112(b)(1):  Guardrails were not provided at locations where employees were exposed to waterside edges, including bridges or gangway-like structures leading to pilings or vessel mooring or berthing installations, which presented a hazard of falling more than 4 feet or into the water.  a) Yard:  On or about April 12, 2023, along the yard boundary there was an opening approximately 69 inches in width, in which employees were exposed to the waterside edge and a fall hazard into the water below.

29 CFR 1917.112(c)(2):  Midrails were not positioned at approximately half the height of the top rail.    a) Maintenance Shop:  On or about April 12, 2023, a stair rail system that measured from the leading edge of the stair tread to the top surface of the top rail was 36" with no midrails installed, exposing employee to a fall hazard.

29 CFR 1917.120(b)(3):  Fixed stairs having four or more risers did not have stair railings or handrails complying with paragraph 1917.112(c)(1):  Guardrails were not capable of withstanding a force of at least 200 pounds applied in any direction at mid-span of the top rail, or at the uppermost point if there was no top rail.   a) Maintenance Shop: On or about April 12, 2023 a flight of fixed stairs with 5 treads and 6 risers did not have a stair rail system installed on the right hand side, exposing employees to a fall hazard.
