29 CFR 1926.453(b)(2)(iv): Employees did not stand firmly on the floor of the basket, or did not sit or climb on the edge of the basket or use planks, ladders, or other devices for a work position: a) US Route 9 West Castleton, NY- On or about June 4, 2019, while laying out anchor bolts an employee climbed out of the man basket and on to the adjacent columns/form work instead of using a ladder for safe access.

29 CFR 1926.501(b)(1): 1926.501(b)(1): "Unprotected sides and edges." An employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a) US Route 9 West Castleton, NY- On or about June 4, 2019, while laying out anchor bolts an employee was exposed to a fall hazard of approximately 18 feet. Employee was not utilizing any means of fall arrest or fall restraint system.
