29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service: On or about 6/4/19, during residential framing activities located at The Reserves at Winfield Farms in Elyria, Ohio, the employer failed to ensure that the synthetic anchors were inspected prior to use. The synthetic anchors were twisted, and showed signs of wear such as being torn and frayed. The anchors were not removed from service. The roof height was about twelve (12) feet from the eve to the ground, and had a slope of 8/12 and 3/12.

Employees were utilizing synthetic beam anchors that were twisted, and showed signs of wear such as being torn and frayed.
