29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required:  a) The employer failed to provide documentation demonstrating the abatement methods and completion as cited in Citation 1 Item 1 for inspecting fall arrest systems prior to use when they were used at The Reserves at Winfield Farms in Elyria, Ohio.    b) The employer failed to provide documentation demonstrating the abatement methods and completion as cited in Citation 1 Item 2 for not ensuring employees were trained on how to recognize and minimize fall hazards located at The Reserves at Winfield Farms in Elyria, Ohio.   c) The employer failed to provide documentation demonstrating the abatement methods and completion as cited in Citation 1 Item 3 for not ensuring employees were trained on how to operate a industrial vehicle when they were used at The Reserves at Winfield Farms in Elyria, Ohio.

29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service:  On or about 6/4/19, during residential framing activities located at The Reserves at Winfield Farms in Elyria, Ohio, the employer failed to ensure that the synthetic anchors were inspected prior to use. The synthetic anchors were twisted, and showed signs of wear such as being torn and frayed. The anchors were not removed from service. The roof height was about twelve (12) feet from the eve to the ground, and had a slope of 8/12 and 3/12.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about 6/4/19, during residential framing activities located at The Reserves at Winfield Farms in Elyria, Ohio, the employer failed to ensure that employees were trained to recognize fall hazards and how to mitigate those hazards. Employees were utilizing synthetic beam anchors that were twisted, and showed signs of wear such as being torn and frayed. The roof height was about twelve (12) feet from the eve to the ground, and had a slope of 8/12 and 3/12.

29 CFR 1926.602(d): The employer did not ensure that each employee operating a powered industrial vehicle had been trained and evaluated as required by paragraph 29 CFR 1910.178(l):  On or about 6/4/19, during residential framing activities located at The Reserves at Winfield Farms in Elyria, Ohio, the employer failed to ensure that the operators of the GEHL RS10-55 GEN:2 rough terrain forklift, were trained in accordance with 29 CFR 1910.178(l)(1) through 29 CFR 1910.178(l)(6).
