29 CFR 1926.20(b)(1): The employer did not initiate and maintain such programs as may be necessary to comply with this part.  a)     On June 4, 2019 at the worksite located at 33445 Lake Road Avon Lake, Ohio: The employer did not have a plan or program to identify job site hazards which assist employees in maintaining a safe working environment.  Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during job site operations, decreasing the probability of employee injury and/or illness.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a)     On about June 4, 2019 at the worksite located at 33445 Lake Road Avon Lake, Ohio: The employer did not ensure that employees had been trained in the appropriate use of fall protection such as, but not limited to, the use of personal fall arrest systems or alternative measures while performing roofing installation.  Employees were exposed to approximately a 15 foot fall hazard from the roof to the ground below during the re-shingling of a low sloped commercial roof.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  a)     On June 4, 2019 at the worksite located at 33445 Lake Road in Avon Lake, OH:, The employer did not ensure employees were protected from falls while performing roofing activities on a low sloped commercial roof.  During this activity, employees were exposed to falls up to 15ft. to the ground below.
