29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: a) On or about 2:50 PM on April 24, 2019, at 451 E Altamonte Dr., Altamonte Springs, FL 32701, an employee suffered a work related injury which resulted in hospitalization with treatment on the same day of the injury. The employer was aware of the hospitalization on April 26, 2019. The employer notified OSHA on May 3, 2019 at 5:41 PM of the work related hospitalization. MACY'S, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1904.39(a)(2), which was contained in OSHA inspection number 1315310, citation number 1, item number 1 and was affirmed as a final order on July 27, 2018, with respect to a workplace located at Route 132 Cape Cod Mall, Hyannis, MA 02601.
