On or about June 12, 2019, Small Work Shop:  Compressed air used for cleaning had not been reduced to less than 30 p.s.i.  The compressed air nozzle was not provided blowout holes or other means to reduce pressure when dead-ended.  The Bostitch compressor, which provided the compressed air, was set at 110 p.s.i.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:    A.  On or about June 12, 2019:  1.  Welding Area:  Three white Belkin 15A 6-receptacle relocatable power taps were connected in series (daisy-chained) to an extension cord, against manufacturer's instructions.  The power taps were mounted to the metal work table and used to power hand tools including two DeWalt DWE 4011 4 1/2" angle grinders; a Makita GD0801C die grinder; a Ridgid R1020 7" angle grinder; a Milwaukee portable Deep Cut Band Saw; a Skilsaw; and a DeWalt DW304P reciprocating saw.   2. Paint Area:  A black Commercial Electric 15A 6-receptacle relocatable power tap was powered through an extension cord, against manufacturer's instructions.  The relocatable power tap was used to power a cell phone charger, a Makita DC18SD battery charger and a motion sensor in the welding area.

29 CFR 1910.304(b)(4)(ii)(B): Where connected to a branch circuit supplying two or more receptacles or outlets, a receptacle supplied a total cord- and plug-connected load in excess of the maximum specified in Table S-4:  A.  On or about June 12, 2019, Welding Area:  The Belkin 6-receptacle relocatable power tap mounted to the welding area table was rated at 15A.  The Ridgid R1020 7-inch angle grinder was listed at 120V, 15A.  Use of the equipment exceeded the maximum ampere rating of the receptacle.  Table S-4, Maximum Cord- and Plug- Connected Load to Receptacle shows a maximum load of 12 amperes on a 15 ampere rated receptacle.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:   A.  On or about June 12, 2019, Welding Area:   1.  A yellow flexible extension cord was used in lieu of the fixed wiring of the building to power relocatable power taps mounted onto the metal welding table.  The power taps powered shop equipment including two DeWalt 4 1/2" angle grinders, a Makita GD0801C die grinder, a Ridgid 7" angle grinder, a Milwaukee Deep Cut Band Saw, a Skilsaw and a DeWalt DW304P Reciprocating Saw.  2.  A flat black extension cord was plugged into a relocatable power tap mounted to a metal Husky tool box.  The cord ran under the tool box, across the wall and behind a cabinet to an orange 3-way splitter.  Another extension cord was plugged into the splitter and went behind wooden shelves at the front of the welding area to power a black 6-receptacle 15A relocatable power tap.  The power tap provided power to a laptop computer and another extension cord that powered a speaker at the back left corner of the welding area.  B.  On or about June 12, 2019, Paint Area:  1.  Two orange extension cords, powered through a Belkin 10-receptacle outlet box on the wall, were used to power an overhead light and a black Commercial Electric 6-receptacle relocatable power tap.

On or about June 12, 2019, Welding Area:  A black flexible extension cord was run from in front of a 2-shelf metal cabinet, through a hole and behind the shelves of a wooden cabinet at the front of the Welding Area.  The extension cord powered a black Belkin relocatable power tap that powered a laptop computer, and a speaker.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):  A.  On or about June 12, 2019, Welding Area:  A yellow flexible extension cord was plugged into a wall outlet, laid across floor and covered with work mat in order to energize a 6-receptacle white Belkin relocatable power tap mounted to the welding table.  The flexible extension cord had been in place for approximately three years and was not regularly inspected.
