29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms: a) 34 Oser Ct. - On or about June 4, 2019 and at times prior thereto, an employee was working from a pump jack scaffold at approximately 20' that had a work bench being used as a top rail for the back side of the scaffold with no midrail, no guardrails on the ends, and no other fall protection in place.

The employer did not provide a training program to train employees working on scaffolds in fall hazards.
