29 CFR 1910.28(b)(6)(i): The employer must ensure each employee less than 4 feet (1.2 m) above dangerous equipment is protected from falling into or onto the dangerous equipment by a guardrail system or a travel restraint system, unless the equipment is covered or guarded to eliminate the hazard. a) On or about 6/5/2019, in the Process Area, employees were exposed to a fall of more than 10 feet into a mixing vessel when the grated cover of the 2'x2' access hatch was bypassed. b) On or about 9/3/2019, in the Process Area, an employee was exposed to a fall of more than 10 feet into a mixing vessel when the grated cover of the 2'x2' access hatch was bypassed. The hatch cover can be opened such that a grated cover is left in place guarding the 2'x2' hole or the hatch cover can be opened such that the grated cover is bypassed.
