29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary.  Location: 60 Coleridge Street, Brooklyn, NY.  On or about 6/4/2019,  a) An employee working from the top tier of a 5 tier high tubular welded frame and the working platform was not fully planked.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross braces were used as a means of access.  Location: 60 Coleridge Street, Brooklyn, NY.  On or about 6/4/2019,  a) A 5 tier high supported tubular welded frame scaffold had no safe means of access.  Employees climbed the frames and cross braces to access to work platforms.

Location: 60 Coleridge Street, Brooklyn, NY.  On or about 6/4/2019,  a)  Employees worked from a 5 frame high supported scaffold which was not equipped with guardrails on the working platform, was not fully planked, was braced to a window grate with a rope for stability and had no safe means of access.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to the lower level.  Location: 60 Coleridge Street, Brooklyn, NY.  On or about 6/4/2019,  a) An Employee working on the top tier of a 5th tier high a supported scaffold and the scaffold was not equipped with guardrails.  The employee was not using any personal fall arrest equipment.  Employee exposed to a fall of 30 feet.
