BD & DB, Inc. dba Donovan and Bauer Auto Group, Chrysler shop Wash bay area, on or about June 5th, 2019 and times prior thereto:  Employees worked with and around corrosive chemicals including, but not limited to,  BAF Industries brand Red Devil Degreaser (C-51), and Whitewall cleaner (C-52) during vehicle cleaning operations.  An employee that performed spraying operations of these chemicals did not utilize eye and/or face protection.

BD & DB, Inc. dba Donovan and Bauer Auto Group, Chrysler shop Wash bay area, on or about June 5th, 2019 and times prior thereto:  Employees were potentially exposed to eye and/or face hazards during work tasks including, but not limited to, spraying and washing off of corrosive chemicals during vehicle cleaning operations.  Company employees were not provided with complete and effective personal protective equipment training in that, company employees did not utilize eye and/or face protection at all times during work operations with corrosive chemicals.

Employees did not have access within the immediate work area to a suitable eyewash/shower station.
