29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:    On or about June 5, 2019, employees were observed working on a single story residential roof without utilizing a fall protection system. This condition exposed employees to fall hazards greater than 6-feet to the ground below.   Jesus Espinoza Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1246862, citation number 1, item number 1 and was affirmed as a final order on August 30, 2017, with respect to a workplace located at 776 Pixton Rd. Horizon, TX 79928 .    Jesus Espinoza Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1211750, citation number 2, item number 1 and was affirmed as a final order on August 3, 2017, with respect to a workplace located at 13043 Wesleyan St. El Paso, TX, 79928.

29 CFR 1926.503(c)(3): Inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill.      On or about June 05, 2019, the employer did not ensure that employees working on a single story roof retained the requisite understanding or skill for safe work practice. This condition exposed employees to fall hazards greater than 6-feet to the ground below.   Jesus Espinoza Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1246862, citation number 1, item number 2 nd was affirmed as a final order on August 30, 2017, with respect to a workplace located at 776 Pixton Rd. Horizon, TX 79928 . 29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about June 5, 2019, employer did not ensure that the ladder side rails extended at least 3 feet above the roof's edge. This condition exposed employees to fall hazards.
