Employees were roofing a multi-story multi-family residential structure. Employees used staple guns to secure ice and water membrane to the roof structure. Eye protection was not used during this operation. This condition exposed the employees to an eye injury hazard.

Employees were roofing a multi-story multi-family residential structure. Approximately two employees were not protected from a fall while working on the second story roof. The employer did not ensure that all employees were protected by guardrail systems, safety net systems, or personal fall arrest systems. This condition exposed the employees to an approximate 21.3 foot fall hazard.
