29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  (a) On or about 5/28/2019  - 6401 Tattersall Park Drive in Hoover AL, an employee was installing Structural Insulated Panels approximately 16 feet 5 inches above the ground without fall protection.

29 CFR 1926.602(c)(1)(ii):No modifications or additions which affect the capacity or safe operation of the equipment shall be made without the manufacturer's written approval. If such modifications or changes are made, the capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly. In no case shall the original safety factor of the equipment be reduced:  (a) On or about 5/28/2019 - 6401 Tattersall Park Drive in Hoover, AL, employees were using a JLG Skytrak to lift Structural Insulated Panels that were suspended from the forks.
