OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crush-by hazards:     a.   On or about June 5, 2019, an employee operating an Kubota RTV 400ci All-Terrain Vehicle (ATV), serial number 24168, in the grass median, on the north side of Interstate 10, westbound, near Mile Marker 161.5, Chattahoochee, FL was exposed to rollover hazards in that the employee did not have adequate training on the safe recovery of an RTV.    b. On or about June 5, 2019 the employer did not assure operators and passengers of Kubota RTV 400ci All-Terrain Vehicle (ATV), serial number 24168, fastened the personal restraint seat belts while operating the RTV's.    Goodwill Industries Big Bend, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard OSH ACT of 1970 Section (5)(a)(1) which was contained in OSHA inspection number 1232634, citation number 1, item number 1 and was affirmed as a final order on October 3, 2017, with respect to a workplace located at Hwy 90/Px Ranch Road Marianna, FL 32446.
