29 CFR 1926.502(h)(1)(v): The safety monitor had other responsibilities which could take the monitor's attention from the monitoring function: a) On June 6, 2019 at the address of 1012 North County Road 260 in Clyde, OH: The employer did not ensure a dedicated safety monitor was designated and that his only responsibility was the monitoring function. The casual use of a monitor created a condition in which employees were performing roofing work at the edge of the elevated surface without the benefit of an active safety monitor. Employees operating in this condition were exposed to a 15 ft. fall to the ground below.
