29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.  Site: 8210 21st Avenue Brooklyn, NY  On or about 6/6/19  a) An employee engaged in hoisting operations from the sidewalk to the sidewalk shed was exposed to head injuries. The employee was not utilizing head protection.

29 CFR 1926.501(b)(3): Each employee in a hoist area was not protected from falling 6 feet (1.8m) or more to lower levels by guardrail systems or personal fall arrest systems.  Site: 8210 21st Avenue Brooklyn, NY  On or about 6/6/19  a) An employee worked on the sidewalk shed hoisting cider blocks was not protected from falling. He was exposed to fall of approximately 13 feet.

Site: 8210 21st Avenue Brooklyn, NY  On or about 6/6/19  a) Employer did not ensure employees were trained in fall protection. An employee working from a sidewalk shed hoisting cinder blocks was exposed to a fall hazard of approximately 13 feet.
