29 CFR 1903.19(c)(1): The employer did not certify to OSHA, within 10 calendar days after the abatement date, that the cited violation had been abated:  (a) 1905 Craley Road, Windsor Pennsylvania - The employer did not certify that the citations had been abated within 10 days of the abatement date, on or about November 5, 2019.      Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a) Porch Roof - To gain access to the porch roof, employees used a portable extension ladder, positioned at the front of the house.  The ladder only extended two feet above the porch roof (upper landing) surface, exposing the employees to the hazard of falling 8.5 feet to the ground below, on or about June 6, 2019.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  (a) Porch Roof - Employees tore off the porch roof, and the employees did not use any fall protection, exposing the employees to the hazard of falling 8.5 feet from the roof to the ground below at the front of the house and 19 feet from the roof to the ground below at the rear of the house, on or about June 6, 2019.  Quality Exterior and Remodeling, LLC was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), which was contained in:  OSHA inspection number 1339960, citation number 1, item number 1 and was affirmed as a final order on May 31, 2019, with respect to a workplace located at 1001 N. 17th Street, Harrisburg, Pennsylvania  17105; and  OSHA inspection number 1319754, citation number 1, item number 1 and was affirmed as a final order on August 13, 2018, with respect to a workplace located at 77 Grant Street, Manheim, Pennsylvania  17545.  Abatement certification and documentation is required within 10 days after the abatement date.
