29 CFR 1910.22(d)(2): Hazardous conditions on walking-working surfaces were not corrected or repaired before an employee used the walking-working surface again. If the correction or repair could not be made immediately, the hazard was not guarded to prevent employees from using the walking-working surface until the hazard was corrected: On or about June 6, 2019, and at times prior thereto, in the conveyor area, employees were exposed to fall hazards due to conditions on the conveyors not being corrected before employees were permitted to access the conveyors to unblock jams.

29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources: On or about June 6, 2019, and at times prior thereto, in the conveyor area, when employees were clearing a package jam while standing on a conveyor belt, the employer did not provide appropriate hardware to implement effective alternative measures, such as the hasp, lock, and key hardware associated with a Push Lock and Key System.
