29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift. On or about June 6, 2019, at 4000 US Hwy 90, Broussard, LA the employer failed to ensure that employees wore fall protection when using the extendable boom aerial lift in order to access the roof of the building. Employees were observed at a height of approximately 24 feet.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury. On or about June 4, 2019, at 4000 U.S. Hwy 90, Broussard, LA the employer failed to ensure that employees were trained in the use of aerial lift/scaffold. Employee were observed using an aerial lift without the use of a fall restraint.
