29 CFR 1903.19(d)(1): The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.   Agustin's Roofing, failed to provide documentation demonstrating the abatement method and completion of the following items as cited:         a) Citation 1, Item 1a: To conduct onsite inspection of unsafe condition at the worksite.       b) Citation 1, Item 1b: Failed to provide fall protection training to employees and provide documents that training was done.  c) Citation 1, Item 2: provide for fall protection for employees working at heights greater than 6 feet in construction.

29 CFR 1926.21(b)(2): The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury.  On and about June 6, 2019, employees reroofing a ranch duplex were exposed to hazards associated with roofing, to include falls from heights and ladders, and struck by injuries when they had not been trained to recognize and avoid or correct unsafe conditions or to understand and follow the OSHA Standards that applied to their scope of work.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.  On and about June 6, 2019, employees were exposed to fall hazards while framing a house when they had not been trained to understand and follow OSHA Standards for fall protection.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.   On June 6, 2019, employees were exposed to falls while reroofing a ranch duplex when:  A) Employees removed the original shingles without any form of fall protection;  B) when the employees set up personal fall arrest systems they did not properly install the anchors or eliminate the slack in their lifelines to prevent falls greater than 6'.
