29 CFR 1926.21(b)(2): The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury. On and about June 6, 2019, employees reroofing a ranch duplex were exposed to hazards associated with roofing, to include falls from heights and ladders, and struck by injuries when they had not been trained to recognize and avoid or correct unsafe conditions or to understand and follow the OSHA Standards that applied to their scope of work.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. On June 6, 2019, employees were exposed to falls while reroofing a ranch duplex when: A) Employees removed the original shingles without any form of fall protection; B) when the employees set up personal fall arrest systems they did not properly install the anchors or eliminate the slack in their lifelines to prevent falls greater than 6'.
