29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: a. On or about June 6, 2019, while working underneath an unsecured truss that was being set and aligned, a framing employee was exposed to a head injury hazard in that he was not wearing a protective helmet. b. On or about June 6, 2019, at the right front corner of the house, an employee handing lumber up to another employee in the trusses was exposed to a head injury hazard in that he was not wearing a protective helmet.

29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents: a. On or about June 6, 2019, at the right front corner of the house, a framing employee on the top plate was exposed to an eye injury hazard in that he was not wearing safety glasses while operating a pneumatic nailing gun.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section: a. On or about June 6, 2019, on the top plate of the first-story walls, while installing trusses and roof framing, five framing employees were exposed to 10-feet to 14-feet fall hazards in that they were not protected by a fall protection system.
