29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  a) Jobsite Located at 2501 Washington Avenue, Philadelphia, PA. 19146: Employees conducting masonry operations on the 2nd level of a scaffold were not adequately trained to recognize and avoid unsafe conditions, including but not limited to; fall from height, struck-by hazards, and electrical hazards.  Condition observed on or about 6 JUN 2019.

29 CFR 1926.451(e)(5)(i): Ramps and walkways 6 feet (1.8 m) or more above lower levels did not have guardrail systems that comply with subpart M of this part -- Fall Protection:  a) Jobsite located at 2501 Washington Avenue, Philadelphia, PA.: The employer did not ensure that employees working from approximately 12 feet from the ground below were protected by a fall protection system, thus exposing employees to fall hazards. The employees were working from pieces of formwork which had been used to form a walkway from the street to the top of the formwork approximately 12 feet above the ground below and did not have a fall protection system or guardrails in place.    Condition was observed on or about 6 JUN 2019.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  a) Jobsite located at 2501 Washington Avenue, Philadelphia, PA: The employer did not ensure that employees working from approximately 12 feet from the ground below, on a scaffold created by placing planks onto the frames at the top of the formwork, were trained in the hazards associated with this type of scaffold in order to control or reduce hazards. Employees are climbing and working atop of the formwork on planks placed onto the framework of the concrete formwork.   Condition was observed on or about 6 JUN 2019.
