29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.    As of  November 12, 2019 , the employer had not submitted to the Agency documents that abatement had been completed for each serious violation indicated by the Agency included in Citation 1 Item1 and Citation 1 Item 2, from inspection number 1406617, issued July 30, 2019 with a final order date of  August 28, 2019.  Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system:  a) 37-39 Dawes St., Springfield, MA: On or about June 3, 2019, four employees were observed roofing this residence without any fall protection, more than 24 feet above the ground below.   Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29CFR1903.19(d)(1).

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:  a) 37-39 Dawes St., Springfield, MA: On or about June 3, 2019 employee were observed using a ladder that did not extend any distance past the upper landing surface (roof) of this residence they were roofing.   Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29CFR1903.19(d)(1).
