29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. On the south side of the work site, employees engaged in roofing activities use pneumatic nail guns and were not wearing eye and face protection. Employees were exposed to struck-by hazards and eye injuries.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. On the south side of the work site, employees engaged in roofing activities on a pitch roof of 4:12 without fall protection were exposed to unprotected fall hazards of approximately 30 feet to the ground below.

29 CFR 1926.602(c)(1)(vi): Section 603(e) of American National Standard Institute, B56.1-1969 Safety Standards for Powered Industrial Trucks, as adopted by 29 CFR 1926.602(c)(1)(vi): When a truck was left unattended, load engaging means were not fully lowered, controls were not neutralized, power was not shut off, brakes were not set, and the key or connector plug was not removed. On the south side of the work site, the employer allowed the forklift with the forks elevated to be unattended while installing roofing material on the roof.
