29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: (a) Facility, 8051 Steubenville Pike, Oakdale, PA: On or about June 6, 2019 the employer had several operators of powered industrial trucks that had not been evaluated at least once every three years. There were a few operators that had not been evaluated since February 6, 2004, July 28, 2005 and December 30, 2005.
