29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: On or about June 4, 2019, the employer failed to notify the Occupational Safety and Health Administration (OSHA) when the employee sustained a severe injury and was admitted into the hospital. OSHA became aware of the injury on June 6, 2019.
