29 CFR 1910.332(b)(2): Employees, who were not qualified persons, were not trained in and familiar with any electrically related safety practices not specifically addressed by 29 CFR 1910.331 through 29 CFR 1910.335, but which were necessary for their safety. On or about June 7, 2019 at 2129 Washington Road, Thomson, Georgia, the employer failed to train unqualified employees on the use of safety related work practices such as, working on or near live parts, de-energizing and testing equipment and LOTO requirements after equipment is de-energized.

29 CFR 1910.333(a)(1): Live parts that operate at less than 50 volts to ground were not de-energized when there was an increased exposure to electrical burns or to explosion due to electric arcs. On or about June 7, 2019 at 2129 Washington Road, Thomson, Georgia, the employer did not ensure that an employee de-energized the circuit providing power to the sign he was servicing. An employee was exposed to live electrical parts after opening the signs frame to install light bulbs.

29 CFR 1910.333(b)(2): While an employee was exposed to contact with parts of fixed electric equipment or circuits which were deenergized, the circuits energizing the parts were not locked out or tagged or both in accordance with the requirements of this paragraph (b) of 29 CFR 1910.333(b)(2): On or about June 7, 2019 at 2129 Washington Road, Thomson, Georgia, and times prior, employees making repairs to electrical components did not lock and tag out the circuits.
