29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):      1) On June 6, 2019, an employee operated a rough terrain fork lift to move sections of a scoreboard.  The employer had not ensured that the employee received training and evaluation to ensure the operator was competent to operate the fork lift safely.  Employees were exposed to crushing hazards, in that the weight of the scoreboard sections were not assessed to ensure the forklift and straps were adequately rated.  The load was moved while unstable, in that eyebolts used to attach the straps bent and broke during hoisting and moving of the scoreboard sections and a defective and damaged tag line was used during moving of a scoreboard section.

29 CFR 1910.184(c)(6): Slings were not securely attached to their loads:  1) On June 6, 2019, employees were exposed to crushing hazards while sections of a large metal scoreboard were hoisted and moved, in that slings were not securely attached to the load.   Eyebolts used to attach straps broke and bent during hoisting and moving of the scoreboard sections and there was no safety latch on the fork lift hook used to attach the sling.
