29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: a)Near doorway: On or about June 6, 2019 and times prior, an employee used an 8" bench grinder to clean up parts or sharpen tools. The adjustable work rest was not adequately adjusted on the left side resulting in a 1 inch opening between the abrasive wheel and the work rest.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: (a) Near Doorway: On or about June 6, 2019 and times prior, an employee operated an eight inch abrasive wheeled bench grinder to clean up parts and sharpen tools. The tongue guard was not adequately adjusted resulting in a 1 1/2 inch opening between the abrasive wheel and the tongue guard.
