OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees that were exposed to a crushing hazard & struck by: a) Warehouse / Rack 6 - Employees are exposed to crushing and struck by hazards when removing palletized items from a rack w/ a damaged vertical uprights, on or about 6/10/19. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: a) East end - Production / New Plant: Employees that are responsible for cleaning up chemicals such as, but not limited to sulfuric acid, hydrochloric acid and sodium hydroxide that may leak out from pipes, are not provided an eye wash station that could provide 15 minutes of continuous flow in the immediate area, on or about 6/10/19.
