An employee conducting paint mixing operations tripped over a cord and while trying to prevent her fall, employee reached out to a gate used to deliver material. The gate swung open and the employee fell approximately 8 feet into a cement hopper. 29 CFR 1910.29(b)(13)(i): The guardrail system opening used around holes that serve as points of access (such as ladderways),  did not have a self-closing gate that slides or swings away from the hole, and was equipped with a top rail and mid-rail or equivalent intermediate member that met the requirements in paragraph (b) of this section:  (a) On or about May 29, 2019, the employer did not ensure that a gate on the mezzanine where paint mixing operations take place was properly secured and had a self-closing gate that swung away from the edge of the mezzanine. This resulted in an employee falling approximately 8 feet into a cement hopper after the employee tripped over a cord and while trying to prevent her fall, reached out to the gate, and the gate swung open.
