Site- An employee working outside a trench shield was not protected from a cave in hazard where the excavation was approximately 9 feet deep. b. Site- An employee was not adequately protected from a cave in hazard of approximately 9 feet where road plates imbedded into the ground were used to hold back the soil. The employer did not have any tabulated data or engineered drawings for the use of road plates as cave in protection.
