Powered industrial truck operators shall receive initial training in the following topics; operating instructions, warnings, and precautions for the types of truck the operator will be authorized to operate. a) Warehouse Area-On or about May 20, 2019, employees did not receive adequate forklift training (Order Picker) related specific to the proper use and inspection of the Self-Retracting Lanyards that were integral to the safe operation of order pickers, therby exposing employees to fall hazards up to 25 feet.

The employers pre-inspection checklist did not encompass inspecting the fall protection equipment that is critical to the safe operation of order pickers, thereby exposing employees to fall hazards up to 25 feet.

29 CFR 1910.302(b)(2): Listed or labeled equipment was not installed and used in accordance with any instructions include in the listing and labeling. a) Warehouse Battery Charging Area - On or about May 20, 2019, and at times prior to, employees were required to charge electric powered forklifts, such as order pickers and electric pallet jacks on a daily basis. The employer failed to ensure that fixed industrial battery chargers were installed in accordance with the manufactures safe installation procedures, in that the battery chargers were installed on and around combustible material such as, but not limited to wood, paper, and plastic, thereby exposing employees to electrical fire hazards.
