29 CFR 1910.213(i)(1): Non-working portion(s) of the blade of bandsaw(s) were not enclosed or guarded: (a) Facility, 30 Curry Avenue, Canonsburg, PA: On or about June 6, 2019 approximately 1 1/2 inches of the non-working portion of the blade on the band saw was not guarded to prevent contact during cutting operations. The guard rests on the fence and can only be adjusted down to approximately 3 1/8 inches above the table. An employee was cutting a pin that was to be approximately 1 1/4 inches wide by 6 1/2 inches long from a block of wood approximately 1 1/2 inches thick and 7 1/2 by 7 1/2 inches square. The employee was cutting the piece when it "jumped" and their right pointer finger made contact with the blade. The employee sustained an amputation of the right pointer finger to the second knuckle.

29 CFR 1910.213(s)(9): Suitable sizes and types of push sticks or push blocks were not provided at the work place: (a) Facility, 60 Curry Avenue, Canonsburg, PA: On or about June 6, 2019 there were no push sticks or blocks available for employees to use when using the Tannewitz band saw to cut wood. An employee was using their hands to keep pressure on and guide a block of wood being cut when the board jerked and the employee's right pointer finger made contact with the blade causing an amputation to the second knuckle.

29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy: (a) Facility, 60 Curry Avenue, Canonsburg: On or about June 12, 2019 the energy control procedure for the band saw did not include a step that will be used to address the energy control of the heat press machine that shares the disconnect with the saw. Electrical box 45-T13-1 controls power to the band saw and heat press and the energy control procedure does not include a step that would notify the operator(s) of the heat press when a lockout was to be performed on the band saw which would affect their operation of the equipment.
