29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:  Worksite located at 606 NE 100th St., Kansas City, Missouri- Employees were exposed to a shock or electrocution hazard in that the cord that was plugged in was not grounded and also had its' outer insulation pulled away and inner wires were exposed.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:   Worksite located at 606 NE 100th St. Kansas City, Missouri- Employees were exposed to fall hazards while applying shingling on a residential roof and was not protected from falling by any means of fall protection.
