29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  (a)  The employer failed to provide documentation to the Agency demonstrating that abatement is completed for the serious violations that were issued by the Agency.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  (a)  Employees conducting drywall operations and smoking cigarettes from a 4th floor patio deck were not protected from falling to the lower level. This exposed employees to a fall hazard in excess of 25 feet.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a)  The employer did not ensure that employees conducting drywall installation work and exposed to potential fall hazards had been trained to recognize and minimize the fall hazards associated with their work.
