On or about June 11, 2019, at the above addressed jobsite, employees performing window washing operations from a boatswain chair were descending from heights greater than three hundred (300) feet above grade.

On or about June 11, 2019, at the above addressed jobsite, employees performing window washing operations from the roof were exposed to falls, of three hundred twenty-one (321) feet, created by the presence of unprotected sides.
