29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: a. At the job site - two attachments used for battery lifting were used with forklifts that were not marked to identify the attachments and show the approximate weight of the truck and attachment combination. Violation observed on or about June 12, 2019. U. S. POSTAL SERVICE WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1910.178(A)(5), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1230132, CITATION NUMBER 01, ITEM NUMBER 001 AND WAS AFFIRMED AS A FINAL ORDER ON 11/15/2017, WITH RESPECT TO A WORKPLACE LOCATED AT 8323 SAND LAKE ROAD, ORLANDO, FL 32819.
