29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:   At the residential construction site located at 8729 NE Shoal Creek Parkway in Kansas City, Missouri:  Employees were exposed to fall hazards.  The employer did not establish and implement an accident prevention program where employees were exposed to fall hazards.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  At the residential construction site located at 10234 Belafonte Ave in Kansas City, Missouri:  Employees were exposed to struck by hazards. Employee was observed installing wood materials using a nail gun without eye/face protection.

Employees were observed installing materials and working at height.  Employees were exposed to a fall distance of greater than 15 feet eave to ground without use of fall protection.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:     At the residential construction site located at 8729 NE Shoal Creek Parkway in Kansas City, Missouri: Employees were exposed to fall hazards. Employees were observed accessing and egressing the roof by way of a portable ladder that did not extend 3 feet past landing.  Employees were exposed to a fall distance of greater than 15 feet eave to ground.
