29 CFR 1910.67(c)(2)(v): A personal fall arrest or travel restraint system that meets the requirements in subpart I of this part was not worn and attached to the boom or basket when working from an aerial lift:  On and about June 11, 2019, at a job site, employees were not wearing a personal fall arrest or restraint system and attached to the basket when utilized an aerial lift ( JLG-1350SPJP/SN-0300129847). The employees were approximately 30 feet above the ground in the aerial lift while placing Tarps on the rooftop.
