On or about June 13, 2019, at the above addressed jobsite, the portable ladder(s) used to access the roof did not extend at least three (3') feet above the upper landing surface, thereby exposing employees to the hazards associated with falls. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
